What your TENS Machine Hire includes:

• Hire of 1 x TENS Machine for 4 weeks

• Pack of 4 self-adhesive electrode pads

• One neck cord

• Set of lead wires

• 1 set of batteries

• 1 birth comb

Everything but the pack of 4 self-adhesive electrode pads and essential oil roller must be returned at the end of your 5 weeks or once your baby is born.

I agree and understand the following:

1. I agree and understand that I am paying $70 for a 4 week hire of a TENS Machine and will pay for the postage cost upon returning

2. I will return everything but the 4 self-adhesive electrode pads and essential oil roller at the end of my 4 weeks or once my baby is born if I'm not able to return every item I understand there will be a fee to help replace the items

3. The TENS machines should only be used in accordance to its instructions

4. The success of TENS in labour is neither guaranteed nor warranted by Journey to Postpartum as experiences may differ

5. TENS Machines must not be used by anyone fitted with a cardiac pacemaker, who has heart disease or suffers from epilepsy. This is not a full list of contraindications, please read full contraindications on the entire manual before using and if you have any concerns whatsoever consult your medical team before use

6. In accordance with the instructions, our TENS must not be used until the pregnancy has reached term (37 weeks onwards)

7. I will check the contents of the hire pack upon receiving it and familiarise myself with the TENS and the instructions before labour begins, and if I have any questions contact Courtney from Journey to Postpartum.

8. If the machine is not used for any reason, this is at the discretion of the customer and a refund will not be issued. Unless the request to refund the machine is made before we send it via post, please contact us ASAP if this is the case for a partial refund.

BILLING

After you sign this contract, you will receive an invoice for the hire of your TENS Machine including postage (if needed).

COLLECTION OR POSTAGE

Collection or postage is available for tens machine hire. The collection of the Tens Machine is from Morisset Park, Lake Macquarie. Collection points are also available from the Central Coast, if you would like to organise collection, please contact me to discuss.

If you are choosing postage please provide your best postal address below. Postage fees will be added to your hire fee, and will also need to be covered upon postal return.

DAMAGE OR UNRETURNED ITEMS:
If there is any damage done to the tens machine during your hire that impacts how it works, I understand that there will be a fee of $200 to replace the tens machine.

If there any unreturned items including the tens machine, lead wires or birth comb I understand that there will be a fee to cover the costs of replacing these items.
